Ultrasound-Guided Dermal Fillers in Colombia

If you are considering hyaluronic acid filler, or you already have filler in place, ultrasound changes what is possible. It lets me see where the vessels are, identify the high-risk zones and place the product with far more precision — instead of working blind. In my practice, every facial filler is planned with real-time ultrasound so the result looks balanced and natural, without taking chances. Patients travel from abroad for this, and I see them in Cartagena, Bogotá and Medellín.

Ultrasound-guided facial fillers: seeing before injecting

Ultrasound-guided fillers are injections in which I use a live image to see what lies under the skin before and during the procedure. That image shows the structures that matter: blood vessels, tissue planes and, in many cases, filler that was placed before.

A conventional injection relies on anatomical knowledge and touch. Ultrasound replaces the guesswork. I can plan the entry point, the depth and the exact plane where the product should sit, so the result is safer and looks more natural.

It matters most in three situations: when you already have filler in place, when we are treating a delicate area, and when precision is the whole point. The point is to inject with a diagnosis behind it.

What ultrasound guidance adds to a dermal filler

Each one adds control over a procedure that is usually done by feel.

Real-time visualization

The ultrasound shows every anatomical structure on screen during the injection: vessels, nerves, muscle and bone.

Lower vascular risk

Mapping the arteries and veins before injecting reduces the risk of vascular occlusion, the most serious filler complication.

Precise product placement

The filler is deposited in exactly the right tissue plane, which improves both the result and how long it lasts.

Assessment first

Before any injection, ultrasound detects previous filler, granulomas, fibrosis or anatomical variations.

Problems caught early

If anything looks wrong during the procedure, ultrasound makes it possible to act immediately and correct it.

Verified result

A final scan confirms the product is distributed correctly and rules out any immediate complication.

Conventional filler vs. ultrasound-guided filler

The difference between injecting blind and injecting while you can see everything on screen.

Aspect Conventional filler Ultrasound-guided
Visualization Injected blind, without seeing internal structures Every structure visible in real time
Vascular risk Risk of vascular occlusion from an unseen vessel Vessels mapped beforehand and avoided
Depth Depth estimated by touch and experience Depth measured in millimeters on screen
Prior assessment No internal assessment before the procedure Ultrasound assessment first: old filler, fibrosis, anatomy
Catching problems Complications found later, once there is damage Verified on screen immediately afterwards

Who is a good candidate for ultrasound-guided fillers?

If safety is your first concern in facial aesthetics, this is the procedure for you.

First time with filler

For patients starting with facial filler who want the most controlled first experience possible.

A bad previous experience

For patients who have had complications, asymmetry or disappointing results with another injector.

Safety above everything

For people who research their options and want the most controlled procedure available.

Filler correction

For patients who need previous filler dissolved or corrected, using ultrasound to locate exactly where it sits.

Traveling from abroad

For international patients who want the assessment and the procedure planned around a short stay in Colombia.

High-risk areas

For fillers in anatomically complex areas: the nose, the nasolabial fold and the periocular region.

How we perform an ultrasound-guided filler

A four-step protocol designed to reduce risk at every stage.

1

Ultrasound assessment

A full ultrasound scan of the area before anything is injected: previous filler, anatomical variations, fibrosis or granulomas.

2

Real-time vascular mapping

Color Doppler identifies and marks the arteries and veins in the area — the angular artery, the facial vein and any other relevant vessel — so they can be avoided during the injection.

3

Ultrasound-guided placement

With that information, Dr. Vives places the filler precisely and safely. In some cases the injection is watched live on screen; in most, the mapping done beforehand guides it.

4

Post-procedure check

A second scan at the end confirms the product is distributed correctly, checks that the vessels are intact and rules out any immediate complication.

Dr. Jorge Vives Gutiérrez

Radiologist and aesthetic physician, with a career built on ultrasound and Doppler imaging. Radiology and facial aesthetics combined for ultrasound-guided dermal fillers. That dual training lets him do something very few specialists in Latin America can: see exactly what lies under each patient’s skin, and inject with the precision of someone who reads ultrasound for a living.

Frequently asked questions about ultrasound-guided fillers

The questions international patients ask most about this procedure.

What are ultrasound-guided facial fillers?

They are dermal filler injections — usually hyaluronic acid — in which the physician uses a high-frequency ultrasound to see the anatomy of the face in real time: arteries, veins, nerves, muscle, bone and tissue planes, throughout the injection. That makes it possible to place the product where it belongs and to keep away from the vessels, where the most serious filler complications originate.

Because most serious filler complications happen when product is injected into a blood vessel or compresses an artery. Injecting blind relies on anatomical knowledge alone — but vessels vary from patient to patient. With ultrasound, Dr. Vives can identify the critical vessels before and during the injection. It is the difference between injecting into anatomy you can see and injecting from memory.

No. Most aesthetic physicians in Colombia and Latin America inject filler blind. There are two reasons. The equipment is a significant investment and requires specific training. More importantly, reading ultrasound images correctly in real time requires a background in radiology or diagnostic ultrasound that most aesthetic physicians do not have. Dr. Vives holds both specializations — radiology and aesthetic medicine — which is where this level of control comes from.

Ultrasound guidance targets the most serious filler complications: vascular occlusion, where filler blocks an artery and can cause skin necrosis; intravascular injection, where product enters a vessel directly and can cause blindness; placement in the wrong tissue plane, which produces visible lumps; and injury to the facial nerves. It also detects long-standing problems from previous filler, such as granulomas, product migration or fibrosis.

Especially if you already have filler. Ultrasound shows Dr. Vives exactly where the previous product sits, which tissue plane it is in, whether it has migrated, whether there is granuloma or fibrosis, and how much is left. That matters because placing new product on top of old material produces lumps, asymmetry and an unnatural look — and many complications happen precisely because the injector did not know what was already there.

Ultrasound-guided fillers with Dr. Vives start at COP $1,500,000 for a single area and go up to COP $4,000,000 for a full guided harmonization. A facial ultrasound consultation starts at COP $300,000, and an ultrasound assessment of previous filler at COP $500,000. International patients can request the equivalent quote in US dollars before traveling. The exact price is set at the assessment consultation.

Dr. Jorge Vives is a radiologist who graduated from Universidad de Cartagena, with his degree validated in Spain and experience in ultrasound and Doppler imaging. He is a member of the Colombian Society of Radiology (ACR), the CMAC expert committee and the AAAM. He speaks internationally on facial aesthetics, ultrasound and intervention, and teaches other physicians this technique. His training as a radiologist lets him interpret ultrasound images in real time with the precision that guiding a facial procedure demands.

Yes. Dr. Vives is based in Cartagena, where he sees patients on a regular weekly schedule at his practice in Bocagrande. He also travels to Bogotá and Medellín on scheduled dates each month and sees patients at a partner clinic in each city, following the same assessment protocol.

Ultrasound-guided filler is performed in all three cities: Dr. Vives travels with his own equipment, so the vessels are mapped before injecting wherever you are treated.

Because Bogotá and Medellín are consultation days rather than permanent offices, dates are confirmed before you travel. Request the next available date through the form and we will confirm the city and the date with you. If you are traveling from abroad, you can also start with an online consultation before booking your trip.
