Ultrasound-guided fillers are injections in which I use a live image to see what lies under the skin before and during the procedure. That image shows the structures that matter: blood vessels, tissue planes and, in many cases, filler that was placed before.
A conventional injection relies on anatomical knowledge and touch. Ultrasound replaces the guesswork. I can plan the entry point, the depth and the exact plane where the product should sit, so the result is safer and looks more natural.
It matters most in three situations: when you already have filler in place, when we are treating a delicate area, and when precision is the whole point. The point is to inject with a diagnosis behind it.

How we perform an ultrasound-guided filler

Ultrasound assessment
A full ultrasound scan of the area before anything is injected: previous filler, anatomical variations, fibrosis or granulomas.
Real-time vascular mapping
Color Doppler identifies and marks the arteries and veins in the area — the angular artery, the facial vein and any other relevant vessel — so they can be avoided during the injection.
Ultrasound-guided placement
With that information, Dr. Vives places the filler precisely and safely. In some cases the injection is watched live on screen; in most, the mapping done beforehand guides it.
Post-procedure check

What are ultrasound-guided facial fillers?
They are dermal filler injections — usually hyaluronic acid — in which the physician uses a high-frequency ultrasound to see the anatomy of the face in real time: arteries, veins, nerves, muscle, bone and tissue planes, throughout the injection. That makes it possible to place the product where it belongs and to keep away from the vessels, where the most serious filler complications originate.
Why is injecting with ultrasound safer?
Because most serious filler complications happen when product is injected into a blood vessel or compresses an artery. Injecting blind relies on anatomical knowledge alone — but vessels vary from patient to patient. With ultrasound, Dr. Vives can identify the critical vessels before and during the injection. It is the difference between injecting into anatomy you can see and injecting from memory.
Do all physicians use ultrasound for facial fillers?
No. Most aesthetic physicians in Colombia and Latin America inject filler blind. There are two reasons. The equipment is a significant investment and requires specific training. More importantly, reading ultrasound images correctly in real time requires a background in radiology or diagnostic ultrasound that most aesthetic physicians do not have. Dr. Vives holds both specializations — radiology and aesthetic medicine — which is where this level of control comes from.
Which complications does ultrasound guidance help prevent?
Ultrasound guidance targets the most serious filler complications: vascular occlusion, where filler blocks an artery and can cause skin necrosis; intravascular injection, where product enters a vessel directly and can cause blindness; placement in the wrong tissue plane, which produces visible lumps; and injury to the facial nerves. It also detects long-standing problems from previous filler, such as granulomas, product migration or fibrosis.
Do I need ultrasound if I already have filler?
Especially if you already have filler. Ultrasound shows Dr. Vives exactly where the previous product sits, which tissue plane it is in, whether it has migrated, whether there is granuloma or fibrosis, and how much is left. That matters because placing new product on top of old material produces lumps, asymmetry and an unnatural look — and many complications happen precisely because the injector did not know what was already there.
How much does an ultrasound-guided filler cost?
Ultrasound-guided fillers with Dr. Vives start at COP $1,500,000 for a single area and go up to COP $4,000,000 for a full guided harmonization. A facial ultrasound consultation starts at COP $300,000, and an ultrasound assessment of previous filler at COP $500,000.
